Community Rehabilitation specifically for seniors…
One of our specializations at Rebalance Rehab is eldercare. We understand the unique needs of seniors and those living in long-term care facilities. They need customized approaches and exercise programs specifically designed to be adaptive to their physical capabilities and any complex medical conditions that require long-term management.
Through our team of highly experienced and specially trained physiotherapists and physiotherapy support workers, we can provide the allied health services that long-term care facilities require.
Over the past few years, our focus has been on optimizing the mobility of seniors in a subacute rehabilitation setting, assisted living and residential care facilities. We also offer manual therapy, as well as holistic, exercise-based rehabilitation techniques that help seniors maintain the activities of daily life, as well as encourage physical activity which can lead to positive gains for seniors, including increased strength, stability, fall prevention, overall fitness and the confidence that comes from movement.
All of our registered physiotherapists are trained to work with special populations, specifically seniors or those requiring complex care.
They will begin by performing a detailed assessment of a client’s mobility and equipment needs, followed by rehabilitation planning. They will customise a treatment plan, then support that with ongoing visits, education for both clients and family, as well as annual conference assessments, where a summary of their recommendations can be made in person, with the family, or through a written report. We can follow that with ongoing rehabilitation program monitoring, ongoing assessment of a client’s mobility status, reviews of wound management, skin integrity management, equipment positioning, as well as wheelchair or other equipment recommendations.
Our Physiotherapy Support Workers
All of our physiotherapy support workers have been working in the field of geriatric care and rehabilitation for several years. They know how to work with clients experiencing limited mobility, especially those living in residential care facilities.
Our caring physio support team will work with clients, one-on-one, to implement the programs designed by our registered physiotherapists.
The dates and times of each session can be pre-booked for the year or done on a month-to-month basis.
Rehabilitation sessions usually focus on mobility, strength and balance through passive range of motion and/or active exercise therapy. The goal of these sessions is to improve or maintain function, whether that’s the ability to walk, to be mobile while outside the facility or to self-propel a wheelchair.
Our physio support workers can also help optimize their seated posture in a wheelchair or to maintain joint mobility for personal care. These sessions are focused on improving a senior’s quality of life and to assist and support the functions of daily living.
